Understanding hyperhidrosis

  1. What it is

    Hyperhidrosis is sweating well beyond anything your body needs for cooling. The sweat glands themselves are normal in number and structure. The nerve signal telling them to fire is overactive, which is why they run in a cool room while you sit still.

  2. Why it happens

    In most people it is primary, meaning there is no illness behind it and the wiring simply runs that way, often from the teenage years and often in families. In a smaller group it is secondary to thyroid disease, diabetes, infection, medication or menopause, which is why the history and sometimes bloods come before treatment.

  3. What you might notice

    Palms wet enough to dampen paper and make a handshake awkward. Soles that slip inside shoes. Underarm patches showing through clothing within minutes of dressing. Sweating that is roughly symmetrical, that stops while you sleep, and that worsens noticeably with stress.

  4. How it is treated

    Strong aluminium chloride preparations used correctly go further than most people realise and are usually the first step. Botulinum toxin injected into the palms, soles or underarms blocks the signal reaching the glands and gives several months of relief. Tablets and other options exist where they suit.

  5. When to get it seen

    If you are choosing clothes, work or social plans around it. This is treated far more successfully than most people assume. Sweating that began suddenly, that affects one side only, or that wakes you at night should be assessed rather than managed at home.
